Medical Marijuana Cards

Pricing:

a. When booking an appointment we ask for a $125 deposit to secure your appointment. This fee will be deducted from your office visit.

b. At your first appointment, you will be charged the balance of $125. This can be paid via Zelle, Venmo, cash, or with a credit card*. Your total fee for the doctor’s visit will be $250.

c. An additional fee of $75 is required by the state registry and can be paid through a debit or credit card, check, or money order.

d. There is a $175 patient transfer/renewal fee when transferring from a doctor outside of the CBD + More umbrella and is due at the time of booking your appointment.

e. A re-certification by the doctor is required by the state every 210 days in order to maintain a valid medical marijuana card.

f. The state registry must be renewed yearly

Wellness Consultation

a. An opportunity to meet one-on-one with Yasmine, the “Head Canna-Goddess”, to better understand your history and discuss individual wellness questions, concerns, and needs as related to your health and well being. We will discuss the various elements and your body’s factors that determine your personal sensitivity. We may also discuss nutrients for wellness, along with diet and lifestyle.

Options for nutrient, vitamin, and mineral supplementation, including recommended dosages, may be covered during the wellness educational consultations. Lab testing and prescription drugs are not available with this service, although we can discuss options for testing that are available to the general public.

Price: $200/hour
